San Diego - Medical Case Manager

Job Posting – Immediate Opening

Position Title: Medical Case Manager – Full time (1.0 FTE) Exempt Position

General Description: Facilitates and actively participates in the delivery of health-related services for clients of the agency.

Supervisor: Clinical Director

Job Duties & Responsibilities: (Illustrative only)
1. Responsible for recruitment of healthcare practioners in private practices, clinics and hospitals to provide medical, dental and allied health services for clients.
2. Orients health practioners to special needs of torture survivors and the collaborative working partnership with Survivors of Torture, International
3. Stays current in the field of torture rehabilitation and serves as a resource to the healthcare community
4. Evaluates clients’ medical status and related needs
5. Participates in developing treatment plans.
6. Assures clients receive needed medical, dental and allied health care services in a cost-effective manner
7. Organizes physician trainings to document the physical effects of torture
8. Assures timely completion and delivery of medical affidavits by physicians
9. Coordinates requests by psychiatrist for donated medications from pharmaceutical companies.
10. Coordinates psychiatrist’s requests for blood work
11. Maintains accurate and complete records
12. Travels to client homes, healthcare providers and various offices as required
13. Engages in self-care activities personally and with agency staff
14. Assists with special projects and other duties as assigned by supervisor

Additional Qualifications:
Minimum Qualifications & Requirements:
• Graduate of an accredited 4-year college in nursing, public health or related healthcare field.
• General working knowledge of case management practices
• Familiarity with treatment issues related to pain, psychological factors and rehabilitation
• Excellent oral and written communication, problem-solving and organization skills
• Experience in working with diverse ethnic backgrounds and language groups
• Working knowledge of basic computer skills including Microsoft office products
• Possess valid Calif. driver’s license, adequate auto insurance and reliable vehicle
• Ability to work independently and as a team member
• Strong commitment to the mission of Survivors of Torture, International
• Ability to speak Spanish, French or Arabic is a plus
